Step 1
Remember to have it with you. It is one of the basic documents you will need.
Step 2
Bring current and previous laboratory test results (most of all: creatinine, urea, potassium, peripheral blood counts, urinalysis).
Step 3
If you have an abdominal ultrasound test results, it is worth bringing it with you. Hospital discharge summary reports may also prove useful. Therefore, it is best to bring all the documents relating to your health with you.
Step 4
Have a list of all medications and supplements (with doses) you are currently taking with you. You may also take your medications with you and show them to the doctor during your visit.
Step 5
Bring blood pressure diary with you, and if you do not have one, start it a few days before your visit. If you have diabetes, you will also need a glycemic control diary.
Step 6
Before visiting the clinic, check how much you weigh, how much fluids you consume and how much urine you excrete per day. This additional information that may be useful.
